Sacubitril/valsartan is a combination medication that works by two distinct mechanisms. Sacubitril is a neprilysin inhibitor, which blocks the breakdown of naturally occurring peptides that regulate the neurohormonal systems, including natriuretic peptides and bradykinin. This inhibition leads to increased levels of these peptides, promoting vasodilation, natriuresis, and diuresis, resulting in reduced blood pressure and improved cardiovascular function. Valsartan, on the other hand, is an angiotensin II receptor blocker (ARB). It blocks the angiotensin II receptors, preventing the vasoconstrictive and aldosterone-secreting effects of angiotensin II. This action leads to further vasodilation, decreased blood pressure, and reduced strain on the heart, thereby complementing the effects of sacubitril and providing additional cardiovascular benefits.
